B. What is the mission of the agency?
The Chris Jensen Health & Rehabilitation Center is committed to providing a homelike environment for those who require long term care or rehabilitation services. We strive to maximize each individualÕs functional independence and to provide compassionate care that respects each personÕs quality of life.
C. What is the population being served?
Ages 18+, but primarily geriatric residents in our 205-bed facility
D. What type of services is provided by the agency?
Short term rehab: medical care, physical therapy, occupational therapy, speech therapy, Long Term Care, palliative and comfort care, specialized Dementia unit
E. What methods of intervention are used (i.e. casework, group work, community organizing, etc.)?
Casework, groupwork
F. Describe
the experience and educational background of the MSW social worker available
for student supervision. (The MSW
is required by our program to meet CSWE accreditation standards.) Attach a
resume.

STUDENT - FIELD PLACEMENT DESCRIPTION
A. Describe the educational experiences available to students. What tasks and responsibilities could be assigned to the student? Differentiate between Field I and II. (This section will determine if you will be assigned Field I or Field II students. You may want to review the requirements for Field I and II in the Field Manual. The Field Coordinator is available for assistance and further discussion).
Admission procedures/discharge planning
Assessments/Interventions: psychosocial, mood, behavior, cognitive
MDS, RAPs, formulation and implementation of care plans
Quarterly and annual care conferences
Resident council/family council
Palliative care/memorial services
Counseling, mediation with residents and/or families
Group work with residents
Interdisciplinary teamwork with staff
B. What types of clients will the student be exposed to?
Persons with one or more of the following: rehab potential, aging issues, dementia, mental illness, developmental disabilities, long term medical needs
C. What resources would students have the opportunity to utilize in performing above tasks? Will office or desk space be made available to the student?
Student becomes a working part of the department, shares office with lead social worker, participates as staff in meetings, inservices, etc.
D. How will supervision be
provided (i.e. availability and frequency of supervision)?
No less than one hour per week of intensive, teaching, individual MSW supervision is needed. However, to meet the educational needs of the students in the optimal fashion, this is probably only the minimum.
Supervisor and student work side by side as mentor/student with student gradually
becoming more independent. Communication, supervision, teaching, etc., are
ongoing.
E. To what extent can students participate in staff meetings relating to cases or projects?
To the extent that meeting times and student are in facility at same time. Many
opportunities for inservices, staff meetings, etc.
F. Will students be able to be exposed to/or participate in administrative meetings, research and evaluation projects (if outside their job descriptions), and policy development tasks?
The nursing home staff is interdisciplinary. The student becomes part of the team
immediately, participating in projects and decisions as their schedule allows
G. Will the student have the opportunity to attend and participate in workshops and training sessions?
Yes, everything that becomes available and fits the studentÕs schedule
H. Does your agency provide an opportunity for students to get experience working with diverse (people of color, lesbian/gay/bi-sexual/transgender, and other oppressed populations) individuals/groups? Please explain.
Only if any particular resident fits into one of those diverse groups.
