Agency Description:	This agency provides quality services that enable self-determination, informed choice, maximum independence and the dignity of risk to persons with developmental disabilities.
Description of Field Activities	Students will have the opportunity to participate in management team meetings, coordinate services with Consumer's Support Network, intakes/interviews with families and consumers, participate in support network team meetings, develop consumer's personal outcomes plan and methods, write quarterly reports, write health and wellness plans, work on compiling and evaluating consumer satisfaction surveys, review policy and procedures, develop inservice trainings, develop respite care checklist.
