Agency Description: SLCHHS is a county human services and public health agency.   Family Outreach is designed to work with families in their homes and communities on a flexible schedule to develop and maintain a cooperative and supportive working relationship with each family. Services are solution-focused, based upon problems identified by the family, and use a variety of techniques and skills to assist the families in resolving their issues.  Family Outreach works with families to acquire basic needs, such as food, clothing, and shelter, and may also work with them to connect with the resources they need to support and sustain themselves in their respective communities.
Description of Field Activities

The student will gain experience in direct practice social work with families and children working with them in stabilizing and maintaining their family unit.  This includes intake, case planning, case managements, referrals, assessments of needs and strengths, strengths-based family intervention, multidisciplinary team assessment, and termination.  The student will have opportunities to work with social workers at other agencies including local schools.  Some court related services are involved in Family Outreach.  Using a “best practices” approach, the student will educate, empower, advocate, intervene and counsel their clients.
