Agency Description:	This agency works to ensure that all children ages 0-21 and their families can lead healthy and meaningful lives. It serves as a liaison with schools in Lake and Cook counties, allocates funds to community partners for service provision, and facilitates community collaboration and systems change.
Description of Field Activities	Students will develop a plan to increase community awareness of the value of collaboration, which includes web site development and public relations; will research funding sources and assist in developing grant proposals to support school based projects promoting the graduation of all children; develop evaluation tools and or satisfaction surveys; coordinate and facilitate student groups; and develop 1:1 connections with individual students.
Approval for Field I, II, Either??	This agency has only been approved for Field II, as there is presently no on-site MSW supervision. (as of 9/9/05)
